We’re excited to welcome you to the UConn Department of Public Health Sciences! As you prepare to begin your graduate studies, this page will guide you through several important steps to ensure a smooth transition into the program. Please review each section carefully and complete all necessary forms and requirements prior to the start of classes.
Our program orientation will take place on Wednesday, August 20, 2025, beginning at TBD. This in-person session is mandatory and provides essential information about program expectations, university policies, and academic opportunities. You’ll also meet your classmates, faculty advisors, and program staff. Additional orientation details will be shared with you via email.

Background Check
Every student must complete and submit a background check form before receiving their university ID badge. -

All students must register for a parking permit—even if you don’t plan to park on campus regularly. -

UConn Health requires proof of immunizations for student enrollment. -

UConn Health mandates several trainings for all incoming students—details and access will be provided. -

Badge
Your student ID badge is essential for campus access and must be obtained during Orientation.

Step 1 – Background Check
Every student must fill out a background check form and be cleared before receiving an ID badge that is necessary to enter buildings, use the library, etc.
The required Background Information Sheet will be emailed to you in your Orientation Packet. The completed form should be returned to publichealth@uchc.edu, as soon as possible. The Program Office will submit the forms for processing, along with payment. If your background check has cleared, you will obtain your ID badge during Orientation on August 20, 2025, at the Administrative Services Building, 3rd floor, Room D3500. It is important that you carry your ID with you at all times on campus.

Step 2 – Parking
All students parking on the UConn Health campus are required to register for a permit (even if you will not park there), park in their assigned locations, and display a valid hangtag.
Parking passes are issued through Parking, Transportation & Event Services at an annual fee ($162), per fall and spring semester ($67.50) or per summer session ($27.00) cost. You will be emailed the Student Parking Permit Request Form in your Orientation Packet which should be returned to publichealth@uchc.edu, along with your Background Information Sheet. You will be instructed to call Parking and Transportation to submit payment over the phone. Your permit will be obtained, along with your ID badge, at Orientation.
On the UConn Health campus, student parking on weekdays is restricted to Garage 1, Levels 4, 5 and 6. After 5 p.m. and on weekends, vehicles with a valid hangtag may park in Lots A1, A4, and A5; West Road; 195 Farmington Avenue (Area 3 only); and the Cell and Genome Sciences Building (Area 3 only). Parking in unauthorized locations and/or during unauthorized times will result in citations. A campus parking map is available at: https://health.uconn.edu/park/forms-and-maps/.

Step 3 – Immunization Requirements
You likely received a medical school form from the Graduate School when notifying you of acceptance to our program. Disregard that form. In its place, use the UConn Health Medical/Immunization Form enclosed with this letter. Once completed, return it to UConn Health’s Employee Health Services (EHS) 860-679-2893 which is indicated on the form.
This form can be completed by your primary care physician. If you cannot get to see your physician, you can make an appointment with Employee Health Service at the UConn Health Center.
VACCINATION and IMMUNIZATION REQUIREMENTS:
- Documentation of two MMR vaccines and two varicella vaccines. If this cannot be obtained immune titers are acceptable for Measles, Mumps, Rubella, and Varicella. If any titers are negative or indeterminate then documentation of booster vaccines are required after the negative titer.
- 2 negative TB skin tests or 1 negative Quantiferon TB Gold blood test are required to be completed within the previous 12 months. If you have a history of a positive skin test then the Quantiferon blood test is required. If you have history of a positive skin test and blood test then a chest x-ray is required within the past 12 months
Please call Central Registration at 860-679-1600 between the hours of 8:00am-4:30pm to register your name, home and current address, emergency contact, and health insurance information.

- 2025 - 2026 Annual Compliance Training
All of these trainings MUST be completed prior to orientation per the Graduate School
You will need your NETID to sign into these trainings.
Interpersonal Violence Training
Graduate students must complete a one-time online interpersonal violence prevention training. A hold is placed on a student’s registration to ensure that you meet this requirement.

Annual Compliance Training
This training addresses requirements pertaining to academic, clinical (patient care), administrative, and certain research environments, and UConn Health mandates completion of this training to meet our regulatory obligations and foster an environment of safety, quality, and compliance.
